Field case management services assist the injured worker who is experiencing lost time from work and/or has need for multiple resources. Kimberly Sellers’ case manager will provide timely in-person initial assessment of the injured worker at which time, an individualized case management plan will be implemented. The case manager will work closely with the injured worker and their employer to determine available and safe transitional or modified duty work. The case manager will assist in coordinating the return to modified, transitional and full duties, and will serve as a resource for the injured worker and employer upon return to work.
The field case manager will serve as the injured worker’s advocate, collaborating with all parties to facilitate and coordinate the physician-prescribed care and services, leading to timely recovery and return to work and wellness.
